New revelations have surfaced regarding the final moments of beloved Friends star Matthew Perry, who died from an overdose of ketamine last October. Court documents reveal that Perry allegedly urged his assistant to administer a “big” dose of the drug shortly before his untimely death.

The detailed timeline of Perry’s tragic final hours came to light in court filings after his live-in assistant, Kenneth Iwamasa, and four others were formally charged on Thursday in connection with the 54-year-old actor’s death.

According to the plea agreement that Iwamasa eventually signed, Perry had been undergoing ketamine therapy for depression in the weeks leading up to his death. On the morning of October 28, 2023, Perry reportedly asked Iwamasa for his first ketamine injection of the day around 8:30 a.m. The assistant, who had worked for the actor since 1994, complied.

Four hours later, Iwamasa administered another dose of ketamine while Perry relaxed at his Pacific Palisades home, watching a movie, according to the court documents. Then, just 40 minutes after the second shot, Perry requested a third injection.

“Give me a big one,” Perry allegedly told Iwamasa before asking him to prepare the hot tub. After administering the third shot within a six-hour span, Iwamasa left to run errands. Upon returning, he found Perry face down in the hot tub.

In the days leading up to his death, Perry had received a total of 27 ketamine injections from Iwamasa, the court filings reveal. The assistant, who had no formal medical training, followed instructions provided by Dr. Salvador Plasencia and Dr. Mark Chavez, two physicians also implicated in the case. Prosecutors allege that these injections, particularly the final three, directly led to Perry’s “death and serious bodily injury.”

Alongside Iwamasa, Plasencia and Chavez, street dealer Erik Fleming and a woman named Jasveen Sangha—known as the “Ketamine Queen of Los Angeles”—have also been charged in connection with Perry’s death. Authorities believe that the fatal dose of ketamine was supplied by Sangha.

Plasencia and Chavez are accused of supplying Perry with approximately 20 vials of ketamine between September and late October, in exchange for around $55,000 in cash. Both have been charged with one count of conspiracy to distribute ketamine in relation to Perry’s overdose.

The charges surrounding Perry’s death have sent shockwaves through Hollywood, as more details about the late actor’s struggles with depression and drug use emerge. Despite his fame and success, Perry had long battled addiction, a fight that tragically culminated in his passing.
